Medical Termination of Pregnancy
Medical abortion - also known as Medical Termination of Pregnancy (MTOP) or pill abortion - is a safe and effective alternative to surgical abortion, where two medications are taken that cause a miscarriage. Dr Tosch has been a certified MTOP prescriber since 2018, and can provide a medical abortion up to 9 weeks (63 days) gestation for patients on the Central Coast of NSW.
The first medication Mifepristone (Step 1) blocks the hormone progesterone, this stops the pregnancy growing and causes the placenta to detach.
The second medication Misoprostol (Step 2) is taken 36 to 48 hours later at home, this causes the uterus to contract and expel the pregnancy.
Medical Abortion is:
Discreet: Looks like you are just having a GP appointment.
Confidential: Fewer people involved. Only the receptionist, doctor, and pharmacy need to know.
Non-surgical: Less-invasive, no cervical or uterine instrumentation, and no additional general anesthetic risks or recovery.
Highly successful: Up to 98% do not require further treatment.
Economical: Usually a lower out-of-pocket gap fee than surgical termination.
Supportive: Dr. Tosch will spend the time explaining the entire process and what to expect, and call you to check on your progress. This ensures you are properly prepared and can pick up and manage any complications early on.
Holistic: Dr. Tosch has a patient-centered approach. She can provide effective contraception going forward that is tailored to your lifestyle and family plans, support you through your abortion, and can manage the other aspects of your health.
Who Can Have a Medical Abortion?
To be eligible to have an MTOP with Dr. Tosch, patients must meet the following criteria:

Be no further than 2 hours from emergency medical care in the 14 days following MTOP
Be checked for chlamydia and gonorrhoea and if present have treatment prior to MTOP (Dr. Tosch can organise a urine test if not already done)
Not have any of the following medical conditions:
Chronic adrenal failure
Bleeding disorders or treatment with anticoagulants (eg. Warfarin, Xaralto, Pradaxa)
Disease that is dependant on steroid medication (severe asthma)

Unstable diabetes or cardiovascular disease
What Happens at the Appointments?
Two long appointments
First Appointment (Planning) Dr. Tosch will take a history, discuss what is involved, check you are eligible to have a medical termination, and give you printed information to take away and read. She will order the medication, and if not already done organise bulk billed blood and urine tests, and a bulk billed ultrasound.
Second Appointment (Treatment) The doctor will discuss your results and confirm that you can have MS-2 Step. You will sign consent forms, be given the abortion medication and pain and anti-nausea medications, and make a plan on exactly when to take the tablets. Dr. Tosch will give you a blood form to check your hormone levels 1 week later and arrange a time for follow up to discuss these results, usually by phone.
Important Information
1) The first two appointments must be booked as long appointments.
2) Your second appointment must be no later than exactly 9 weeks pregnant, using the dates on your scan not your period.
3) Around 2% of people having MTOP will need surgical D&C for heavy or prolonged bleeding, or for continuing pregnancy. Dr Tosch will explain the specific risks of MTOP, and can help you access this in the rare case it is required.
4) It is strongly recommended that you have an adult support person present during the process.
5) You must have a blood test to check your hormone levels 1 weeks later.
